死を思うからこそ、不安がらずにシンプルな決断ができる。そんなジョブズ氏が、実際に自身に死が近づいてきたその時を語ります。
About a year ago I was diagnosed with cancer. I had a scan at 7:30 in the morning, and it clearly showed a tumor on my pancreas. I didn't even know what a pancreas was. The doctors told me this was almost certainly a type of cancer that is incurable, and that I should expect to live no longer than three to six months. My doctor advised me to go home and get my affairs in order, which is doctor's code for prepare to die. It means to try to tell your kids everything you thought you'd have the next 10 years to tell them in just a few months. It means to make sure everything is buttoned up so that it will be as easy as possible for your family. It means to say your goodbyes.

ジョブズ氏はこの講演の1年前に癌と診断されました。ジョブズ氏から見つかった"a tumor on my pancreas."（膵臓の腫瘍）は、医師から"this was almost certainly a type of cancer that is incurable"（ほぼ確実に治らない癌）だと告知されます。
さらには、"I should expect to live no longer than three to six months. "（自分が3～6か月以上生きることは期待できない）とも言われます。そのときジョブズ氏は"I didn't even know what a pancreas was."（膵臓がなんなのかも知らなかった）と述べています。それくらい唐突でした。
そんな告知に加えて、医師からは家に帰って"get my affairs in order"（物事を整理する＝身辺を整理する）よう言われました。ジョブズ氏はこの言葉を"prepare to die."（死に備えろ）という、医師の婉曲表現と解しました。
それは、自分の子供たちに"everything you thought you'd have the next 10 years to tell them"（あと十年かけて伝えようと思っていたすべてのこと）を"in just a few months"（たった数か月）で伝えなければならないということです。
またそれは、"to make sure everything is buttoned up"（すべてのことを確実にやめる＝すべてのことをきちんと片づける）ことを意味します。なぜなら、" it will be as easy as possible for your family."（家族ができる限り安心していられる）ために必要なのです。
そして、"It means to say your goodbyes."（別れを告げなさいということを意味している）のです。
最後の部分では、"It means"が連続します。それくらい衝撃が大きかったことが伝わります。また、
「死に向けた準備」というのは、思いのほか心残りがあり、そして限られた時間の中で精いっぱい生きなければならないことを、ジョブズ氏の言葉は痛切に表しています。
